Scope
This policy explains how Pure Wellness & Vitality LLC handles personal information collected through https://purewellnessvitality.com and related public website features we operate.
Protected health information handled in our healthcare practice is also governed by applicable health privacy law and our Notice of Privacy Practices. This website policy does not authorize a disclosure prohibited by those laws or reduce the protections in that notice.
Information we collect
We collect information you provide when you contact us, request an appointment, or otherwise interact with available website features. Depending on what you submit, this may include your name, contact information, appointment preferences, and the contents of your message. Information collected during clinical intake or care may include health history and other medical information and is subject to applicable health privacy protections.
Our website and hosting services may also receive technical information such as IP address, browser and device information, visited pages, timestamps, and referral sources. The specific cookies and other technologies in use are disclosed below.
Please do not send detailed medical information through public contact forms or social media. Use the designated patient portal or another channel identified by the practice.
Cookies, analytics, and embedded services
We use the following cookies and similar technologies on this website:
Essential cookies. These are needed for the website to work, keep it secure, and remember your cookie choices. They cannot be turned off in our cookie settings.
Analytics. We use Google Analytics, provided by Google LLC and loaded through Google Tag Manager, to understand how visitors use our website. It may receive information such as pages visited, time on page, browser and device type, approximate location derived from IP address, and referral source.
Advertising. We use the Meta Pixel, provided by Meta Platforms, Inc., and Google Ads conversion and remarketing tags, provided by Google LLC. These tools may receive information such as pages visited, buttons clicked, IP address, and cookie or device identifiers. We use them to measure the performance of our advertising and to show our ads to people who have visited our website.
Embedded services. Our appointment scheduler is provided by Practice Better. Information you enter in the scheduler is received by Practice Better to arrange your appointment and is also subject to its privacy policy. Embedded YouTube videos, provided by Google, may set cookies when played. Our website uses Google Fonts, which receives your IP address to deliver fonts.
Analytics and advertising technologies run only after you agree through our cookie banner where required by law. You can review or change your choices at any time using the Consent Preferences button on our website. The cookies we use and how long they last are listed in our cookie settings. You can also use Google’s ad settings and Meta’s ad preferences to control personalized ads. We treat a Global Privacy Control (GPC) signal from your browser as a request to opt out of the sale or sharing of your personal information for that browser.
Browser controls may allow you to block or remove cookies; some features may then function differently. Website tracking disclosures and consent do not substitute for legally required authorization to disclose health information.
How we use information
We use information for purposes permitted by law, including responding to inquiries, arranging appointments, operating website features, facilitating requested services, managing payments, maintaining security, and meeting legal obligations. Promotional communications are sent only as permitted by law and subject to applicable consent and opt-out requirements.
When we disclose information
We may disclose information to service providers that perform authorized functions, such as hosting, scheduling, communications, records administration, or payment processing, subject to applicable legal requirements and required contracts.
Certain billing and pharmacy coordination services are handled through our affiliate, Pure Wellness & Rejuvenation LLC. Disclosures to that entity are limited to legally permitted purposes and subject to required safeguards. Affiliation alone does not authorize unrestricted sharing of patient information.
We may disclose information when lawfully required, to respond to valid legal process, or to address security incidents or legally recognized threats. Any transfer in a business transaction must comply with applicable privacy law and required safeguards.
Our use of advertising technologies from Meta and Google may be considered a “sale” or “sharing” of personal information for targeted or cross-context behavioral advertising under some state privacy laws, including California law. We do not sell personal information for money. You may opt out by using the Do Not Sell or Share My Personal Information link on our website or by updating your cookie settings. Analytics and advertising technologies do not run unless you accept them through our cookie banner, and your choice applies to the browser and device you use. We do not knowingly sell or share the personal information of consumers under 16 years of age.
Payments and third-party services
Payment transactions may be processed by a designated payment provider or affiliated billing entity. Any billing identity and applicable charges will be identified through the payment or service agreement process. We do not request payment card details through public contact forms.
Links to external websites or services may lead to organizations with their own privacy policies. Review those policies when using their services. Our obligations for information disclosed by our practice remain subject to applicable law.
Email and text messages
Where permitted and consistent with your communication preferences, we may use email or text messages for appointment, administrative, or service communications. Ordinary email and SMS may have privacy risks. A public website submission is not blanket consent to promotional messages or sensitive medical communications.
You may unsubscribe from promotional emails through the provided link or contact us. If we send text messages, follow the instructions in those messages to opt out, including replying STOP where supported. Message and data rates may apply. Opting out of marketing does not necessarily stop legally permitted essential service communications; contact us to discuss communication preferences.
Security and retention
We use reasonable administrative, technical, and physical safeguards appropriate to the information and applicable legal requirements. No online transmission or storage method eliminates all risk.
We retain information for the periods needed for the purposes described here and to satisfy applicable records-retention, legal, accounting, and dispute-resolution requirements. Medical records may need to be retained even after services end or you request deletion.
Your choices and privacy requests
You may contact us to ask about information we maintain or request access, correction, deletion, or other rights that apply under the law governing your information. We may verify your identity and evaluate legal retention requirements or other permitted exceptions before responding. Rights concerning protected health information are explained in our Notice of Privacy Practices.
Where applicable law provides an appeal or authorized-agent process, we will provide information about that process in response to your request. This policy does not limit additional rights available under applicable law.
Children
Our public website is not directed to children under 13. Parents or guardians should use the practice’s designated process when arranging care for a minor, if that service is available. Contact us if you believe a child under 13 submitted personal information through the public website without appropriate authorization. Minor consent and record-access rights are governed by applicable law and are not determined by this website policy.
Changes and contact
We may update this policy to reflect lawful changes to our practices or requirements. We will post the revised policy with its effective date and provide additional notice or obtain consent where required. An update does not retroactively authorize a use that requires consent.
