Website operator and scope
These Terms of Use govern use of https://purewellnessvitality.com, operated by Pure Wellness & Vitality LLC under the Pure Vitality brand. By using this website, you agree to these terms to the extent permitted by law. If you do not agree, please discontinue use.
These terms concern website use. Clinical care, telehealth consent, subscriptions, payments, and other services may require separate agreements presented before enrollment or treatment. Your statutory patient rights remain in effect.
Emergency care
This website, appointment forms, email, text messages, and patient portal messaging are not emergency services and are not monitored continuously. If you have a medical emergency, call 911 or go to the nearest emergency department.
Website information and the care relationship
General website content is educational and does not constitute an individualized diagnosis, treatment plan, or medical advice. Do not delay necessary care because of website information.
Browsing the website, requesting information, or participating in an introductory consultation does not by itself establish a treating provider-patient relationship. A relationship is established through the practice’s applicable intake, consent, acceptance, and clinical evaluation process consistent with state law.
Eligibility and telehealth
Pure Vitality currently accepts patients in Arizona, California, Colorado, Florida, Pennsylvania, and Washington. Care is available only where the treating clinician is authorized to practice and the practice offers the requested service. You must accurately disclose your identity, age, and physical location at each encounter. A clinician may be unable to provide care if you travel to another state or jurisdiction.
Telehealth has limitations, including the absence of a full in-person examination and the possibility of connection problems. A clinician may require laboratory testing, an in-person assessment, referral, or emergency evaluation. Eligibility and consent requirements are addressed during intake.
Treatment and prescriptions
Treatment decisions depend on the clinician’s assessment, medical history, risks, applicable law, and standards of care. Scheduling, paying for a visit, or joining a membership does not guarantee any diagnosis, medication, prescription, dose, refill, or outcome.
Prescriptions are issued only when medically appropriate and legally permitted. Pharmacies independently determine whether they may fill a prescription under applicable requirements. Medication availability and prices may change. Any compounded medication, if lawfully prescribed and dispensed, is not FDA-approved and is subject to applicable compounding requirements. Individual informed-consent discussions remain necessary.
Fees, memberships, and affiliated billing
Prices and what a service includes are disclosed through the applicable service or enrollment process. Unless expressly included, medication, laboratory, pharmacy, or outside-provider charges may be separate.
Membership billing frequency, recurring-charge authorization, cancellation procedures, refunds, and any visit cancellation or missed-appointment fees are governed by the policies and agreements disclosed before purchase. These website terms do not establish a new cancellation fee or refund restriction. Changes to charges or recurring arrangements require applicable notice and consent.
Certain billing and pharmacy coordination functions are handled through Pure Wellness & Rejuvenation LLC, an affiliated entity. The applicable billing identity will be disclosed through the payment process or service agreement. Contact the practice if a charge or merchant name is unclear.
Your responsibilities
Provide accurate information, protect your account credentials, and promptly report suspected unauthorized access. Do not submit another person’s information without lawful authority.
Do not use the website unlawfully, impersonate another person, introduce malicious software, attempt unauthorized access, interfere with service operation, or collect other users’ personal information without authorization. Do not use automated tools in a way that compromises website security or availability.
Privacy and communications
Review our Website Privacy Policy and Notice of Privacy Practices for applicable information-handling practices. Website use is not blanket consent to marketing, recurring billing, or disclosures requiring separate authorization. Public forms and ordinary messages should not be used for urgent or detailed clinical matters unless the practice specifically directs you to an appropriate channel.

Website text, designs, images, logos, and other materials belong to Pure Vitality or their respective owners. You may view or print materials for personal, noncommercial use. Other uses require permission unless the law permits them.
External links are provided for convenience. The practice does not control third-party websites or guarantee their availability or content. A link does not necessarily imply endorsement.
Availability and limitations
We make reasonable efforts to maintain useful, accurate website information but cannot guarantee uninterrupted access or that all general content will always be current. To the extent permitted by law, public website content is provided as available without warranties about website performance or fitness for a particular purpose.
Nothing in these terms disclaims a clinician’s professional duties, waives a claim that cannot legally be waived, limits rights relating to medical care or privacy, or excuses liability that applicable law does not allow to be excluded.
Changes and contact
We may revise these terms and identify the revision through an updated effective date. Material changes will receive additional notice where required. Revisions do not retroactively change an existing clinical or payment agreement without any consent required by law.
If a provision is unenforceable, the remaining provisions continue to apply to the extent permitted by law. Mandatory protections under the law applicable to you remain in effect.
